W-4 Form:
Please use your name as written on your current social security
card.
I-9 Form:
When hired for a new job, employees are required to prove that they
are legally entitled to work in the United States.
Employers are required to verify the identity and eligibility to
work for all new employees. An Employment Eligibility
Verification Form (I-9 Form) must be completed and kept on file by the employer.
Proof of Personal Auto Liability Insurance:
All Consultants
who operate their personal vehicles on company business, or on the
property of a GSA Client, will be required to furnish a
copy of a valid
driver’s license and proof of vehicle liability insurance with
minimum limits of $250K/$500K/$100K. Before you commence your
assignment, please have your insurance provider fax or email a
Certificate of Insurance or a current copy of your policy

PA Residency Verification for Local Tax
Withholding:
PA Residents only:
Please fill out the top portion of this form (resident
location). This will determine in which municipality
your local
taxes will be withheld.
Timesheets:
Keep track of your daily hours on the time sheet. Have the Manager
sign off every week. Please Email (or fax) your time sheet at the
end of
the week/before you leave for the weekend so we'll have it by 9:00
am Monday morning.
Failure to fax or email your time sheet or
no signature from Manager can result in late payment.
Some of our clients require hours to be entered into
their web-based timesheet. Please also submit a GSA timesheet as
instructed above
for our records.
Paychecks:
Temporary employees of GSA Financial Staffing are issued biweekly
(every other Wednesday) with paychecks mailed out from our
Blue Bell
office.
If a background check /
drug screen is requested, instructions will be emailed to you.
Please fax or email (preferred) the completed W-4, Residency
Verification (if applicable), proof of auto insurance, and I-9 with a copy of
two forms of
identification. If a background and/or drug screen is
requested, please also send back the necessary forms. If you have any
